# Job Description:
Duties:
Person in this position will provide outreach, case finding and mentoring for at-risk youth in his/her assigned geographic area. His/her primary function is to accompany identified youth through the care system from the point of initial contact, counseling, and into the system of care. Essential Job Functions include those listed below. Other duties may be assigned as needed.
Specific Duties and Responsibilities are (but not limited to):
* Help Identify at-risk youth provide counseling, and accompany at-risk youth to medical or other PREP related appointments.
* Provide direct supervision of adolescents, insuring compliance with program rules, and WestCare policies and procedures.
* Supervise clients.
* Facilitate meetings and other groups.
* Assist with intake and orientation of clients.
* Complete support documentation.
* Utilize approved crisis intervention techniques to assist with de-escalation for clients-ensuring safety of all.
* Make home visits as needed for adherence education, tracking and follow-up.
* Accompany youth through the system of care, providing social and emotional support so as to minimize the number of those youth who are “lost to follow up.”
* Follow up with youth who miss medical appointments and assist in rescheduling appointments when needed.
* Maintain youth in care through peer mentorship activities and making necessary referrals to support services such as mental health counseling, substance abuse counseling, emergency housing assistance, employment training, and others as deemed appropriate.
* Assure confidentiality of records as required by state and federal statutes.
* Assist in coordinating transportation for youth to get to and from their medical and other PREP related appointments and support groups.
* Work in collaboration with the Program Coordinator to co-facilitate support groups and other activities for youth enrolled in the PREP program.
* Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation.
* All other duties as assigned.

* Maintenance of individual participant files in accordance with agency’s policies and procedures, including documentation of vocational/educational and other employment activities as well as all other pertinent documentation and ensure files are securely stored.
